If an adopter fails to complete the questionnaire, a staff member or volunteer will follow up with a phone call. If an adopter reports a specific problem, an adoption counselor will provide advice over the phone and send literature as well.
This helpful survey not only keeps the rescue in touch with adopters, but also increases the likelihood that animals will become permanent additions to their new households.

Downloaded copies can be emailed to [email protected] or brought to one of our adoption events.
And please don't forget to send us (or post on our facebook Chicago Pet Rescue Volunteers/Fosters/Alumni group) photos of your adopted pets! These are just a few of our rescued animals that were adopted and are now living happily in their forever homes.
